Report Concern to Board

Any member of the Gator Swim team (parent, coach, swimmer) who has concern that needs Board attention, submit for review this form

Examples of issues that may deem reporting:

• Gator swimmer not adhering to the Gator Code of Conduct
• Parent not adhering to the Parent Code of Conduct
• Opposing teams display poor sportsmanship to our swimmers
• Concerns with staff, coaches or lifeguards

Issue Review Process

1. When an issue is submitted, the President will be notified via email automatically.

The President will connect with the Treasurer for discussion.

President & Treasurer will deem if the concern needs immediately full Board review

2. The President will respond to the person who submitted the issues within 48 hours with next steps. If the president is unavailable, the Treasurer will respond.

3. All issues will be documented and included in the next Board meeting agenda for full Board awareness
